What Causes Hair Loss?

Hair loss can have many different causes, and identifying the right one is the first step to effective treatment. The type determines the appropriate treatment:

Androgenetic alopecia (male and female pattern hair loss) is the most common cause of a hereditary condition driven by the sensitivity of hair follicles to dihydrotestosterone (DHT), a metabolite of testosterone.

In men, it produces the characteristic M-shaped recession and crown thinning of the Hamilton-Norwood scale.

In women, it typically produces diffuse thinning across the crown with preservation of the frontal hairline, the Ludwig pattern. Androgenetic alopecia is progressive without treatment, and the follicles gradually miniaturise and cease producing visible hair.

Telogen effluvium is diffuse, often sudden shedding caused by a systemic shock that shifts a large proportion of hairs into the resting (telogen) phase simultaneously. Common triggers include significant physical or psychological stress, childbirth, rapid weight loss, nutritional deficiency, thyroid dysfunction, or major surgery. It is typically temporary and reversible once the trigger is identified and addressed, but can become chronic if the underlying cause persists.

Hormonal hair loss in women is due to oestrogen and progesterone supporting hair growth; when these decline during perimenopause, postpartum, or with certain contraceptive changes, diffuse shedding can result. Thyroid disorders, both hypothyroidism and hyperthyroidism, are also common hormonal drivers of female hair loss that are frequently overlooked. Female pattern hair loss requires hormonal assessment alongside scalp treatment.

Alopecia areata is an autoimmune condition in which the immune system attacks hair follicles, producing patchy, well-defined areas of hair loss. It can affect the scalp, brows, lashes, and body hair. There is no single answer, as the most suitable approach depends on the type, cause, and stage of your hair loss. For male androgenetic alopecia, prescribed oral and topical medication is commonly used alongside treatments that stimulate follicular activity, such as Regenera Activa or PN Hair. For significant established hair loss, FUE hair transplant is the only option that permanently restores hair. For female hair loss, a hormonal assessment is the essential first step before any treatment is recommended. In most cases, a combination protocol addressing the underlying cause, follicle stimulation, and scalp health together produces better outcomes than any single treatment alone. It depends on the type and stage. Early androgenetic alopecia, where follicles are miniaturised but still present, can often be reversed partially or significantly stabilised with appropriate treatment. Severe cases of hair loss where follicles have been permanently lost cannot be reversed with non-surgical treatments; FUE transplant is the only option for these areas. Telogen effluvium is typically reversible once the trigger is identified and addressed. Hormonal hair loss in women responds well to treatment when the hormonal driver is identified and managed. The earlier treatment begins, the more follicle activity there is to preserve and stimulate.

Female hair loss has multiple causes. The most common are androgenetic alopecia (female pattern hair loss), telogen effluvium triggered by stress, childbirth, or significant weight loss, thyroid dysfunction, nutritional deficiencies such as iron deficiency anaemia, and hormonal changes during perimenopause. Because the causes vary significantly, a proper medical assessment including blood tests is essential. Treating the wrong cause produces no result, which is why our doctors assess each patient individually before recommending any treatment.

Because of the hair growth cycle, most treatments require 3 to 6 months before visible improvement is apparent. New hair growth must progress through the anagen phase and grow long enough to become visible. Patients who stop treatment at 2 months because they don’t see improvement are typically stopping just before results would have become visible. Patience with the timeline is clinically important.

The transplanted follicles themselves are permanent; they are harvested from the DHT-resistant donor area and retain this resistance in their new location, meaning they continue to grow for life. However, the surrounding native hair in the recipient area continues to be subject to androgenetic miniaturisation if the underlying hair loss is not medically managed. This is why most hair transplant doctors recommend continuing medical treatment after a transplant to preserve the native hair around the transplanted grafts and prevent the transplanted area from being surrounded by progressively thinning native hair over time.

Both Regenera Activa and PRP (platelet-rich plasma) use the patient’s own biological material to stimulate hair follicles, but through different mechanisms. PRP uses concentrated platelets from a blood draw to deliver growth factors to the scalp. Regenera Activa uses micro-biopsies from the scalp itself, harvesting progenitor cells and the extracellular matrix components that specifically support follicular stem cell activity, alongside growth factors. Because the harvested material is scalp-derived rather than blood-derived, the regenerative signals are more targeted to follicular biology. Clinical evidence supports Regenera Activa’s efficacy, and it requires only a single session rather than the multiple sessions typically needed for PRP.
