What Are Wrinkle Fillers?

Wrinkle fillers are gel-like substances used to help smooth out the lines and wrinkles in your skin. Fillers can also be used to liftsculpt or volumise, depending on the type of filler used.

Wrinkles appear as you age. This is a normal and natural process. It occurs as you lose fat and proteins in your skin, resulting in your skin sagging and wrinkling. Wrinkle fillers are injected into wrinkled areas of your skin to replace what volume you’ve lost, to plump up and smooth out the wrinkles and lines.

Wrinkle fillers are typically made of hyaluronic acid – a substance also found in our skin and connective tissues.

Do Wrinkle Fillers Really Work?

Yes. Fillers are a proven way to treat facial and even body wrinkles and lines.

But fillers are not the first line of treatment we employ at SL Aesthetic Clinic to address wrinkles and lines. Fillers are reserved for when the wrinkles have become static; they appear even without facial movement. More will be shared later in the article.

Can Fillers Prevent Wrinkles?

No, fillers cannot prevent wrinkle formation. Botox can delay the formation of wrinkles, but wrinkle formation is part and parcel of the ageing process. There is currently no technology in the world today that can claim to prevent this.

In particular, the way hyaluronic acid filler treatments work is largely mechanical and cannot prevent the biological process of wrinkle formation.

Wrinkles appear due to repeated skin folding in the same area, or when your skin loses fat and proteins due to certain processes (i.e. sun damage, gravity, genetics, smoking, poor nutrition, environmental pollution). Fillers simply replaces the volume loss and helps smooth and level out wrinkles and lines.

Collagen-stimulating fillers such as Ellansé work differently by increasing your skin’s collagen production. Your doctor will determine which type of filler works best for you.

Fillers cannot stop the ever marching tune of time – but they can help you age gracefully, and look younger than your peers who have not done fillers or any anti-ageing work before.

What Wrinkles Can Fillers Help With?

At SL Aesthetic Clinic, we use fillers to treat static wrinkles and lines.

Wrinkles are largely categorised into 2 types:

  • Dynamic wrinkles caused by repeated facial movements
  • Static wrinkles that appear with age. These appear even without movement. Dynamic wrinkles can also become static wrinkles over time.

  • Smile lines at the corners of your mouth
  • Lip wrinkles/smoker’s lips
  • Horizontal forehead lines (worry lines)
  • Neck lines
  • Cheek lines

What Is The Best Filler For Wrinkles?

There isn’t a “best” filler for wrinkles. But there are fillers that are more suitable for wrinkles than others.

Fillers best for wrinkles have 3 main characteristics:

  1. A low to medium “G prime”
    The gel hardness and how well it withstands pressure and deformation. Because wrinkles are close to the surface and the skin is expected to move, the gel must be soft enough to allow such movement naturally.
  2. A low to medium cohesiveness
    How well the gel sticks together or spread. Because wrinkles require minute doses of filler, the gel needs to be easily extruded into such a minute area.
  3. A low number of cross-linking
    As wrinkle fillers are required to be “soft”, there is a low number of cross-linking, which means they do not last as long compared to “harder” fillers.

How Are Wrinkle Fillers Performed?

  • After a thorough assessment of your suitability during a detailed consultation, the doctor may numb the area with topical anaesthetic (numbing cream). Numbing is typically not required as fillers already come with lidocaine anaesthetic and are bearable for most patients, but remains an option if required.
  • The doctor will create an injection entry point with a tiny prick of a needle.
  • Then, filler is injected into the wrinkle using a blunt cannula needle. It is a thin and flexible needle that allows the doctor to manoeuvre and place the filler throughout the entire length of the wrinkle.
  • And voila, treatment is complete in less than 30 minutes. Results are immediate as the filler smooths out the wrinkle.

How Long Do Wrinkle Fillers Last?

Results with wrinkle fillers typically last between 3 to 6 months, depending on the type of filler used and location of the injection.

Sometimes, results may last longer than expected as collagen production is stimulated in the area, which increases its longevity.

Are Wrinkle Fillers Painful?

No. Most patients find wrinkle filler treatments to be bearable.

  • Wrinkle fillers are injected near the superficial layer of the skin. This means there’s a lower chance of coming into contact with nerves that go deeper into the skin.
  • Wrinkle fillers contain lidocaine, an anaesthetic, which numbs the treatment area.

To enhance comfort, we use a vibrating tool that distracts patients from any sensations of pain. Where appropriate, we may also use a blunt-tipped cannula instead of a traditional needle for the injection. The cannula technique can reduce pain, bruising, and swelling as it tends to cause less trauma to the tissue compared to a sharp needle.

Are Wrinkle Fillers Safe?

Yes. Wrinkle fillers are safe when performed by a trained professional.

As wrinkle fillers are injected superficially, the treatment avoids many of the risks commonly associated with filler injections such as vascular occlusion. Plus, the fillers can be dissolved if the result is less than satisfactory.

What Is The Difference Between Using Botox Or Fillers To Treat Wrinkles?

Botox is generally injected into muscles to weaken them, whereas fillers are injected into various layers of the skin to lift and support it through restoring facial volume. In short, botox and fillers are injected into different parts of the face, they provide different effects, and they are not interchangeable.

Botox are often used for dynamic wrinkles to limit muscle activity, while fillers are preferred for static wrinkles.

Most patients require less than 1ml of filler to treat wrinkles. This is because most wrinkles are very fine and narrow, which require a small amount of filler.

However, when it comes to deep creases such as the nasolabial folds, you may require more fillers. Treatment of nasolabial folds may also require a combination treatment with other technologies such as Ultherapy/HIFUlasers, and Thread Lifting.

Results with wrinkle fillers are immediate. You will see improvements to your wrinkles, and they will become less noticeable or apparent.

As with any injection treatment, there is a small chance of bruising, redness, or swelling. These are normal and will dissipate in a matter of hours to a day or two.

Because wrinkle filler treatments are minimally invasive and performed with shallow injections, side effects, if any, are mild and temporary. These can include, but are not limited to:

  • Redness of the injection spot. Dissipates within a few hours.
  • Redness of the skin around the injection site. Dissipates in a few hours to a day or two.
  • Tyndall effect – a bluish discolouration seen under the skin. Reversible by dissolving the filler.
  • Dissipates in a few hours.
  • Dissipates in a day or two.
  • Dissipates in a few days’ time.
  • Smooths out in a few days as the filler integrates into the skin tissue.
  • Acne breakout. A potential side effect that occurs when injections are carried out on skin with an active acne, but can also occur when there are no visible acnes.
  • Unlikely to occur when performed with a hygienic protocol.
  • Filler migration. A potential side effect if injected too shallow or the area was massaged post-treatment. For this reason, we do not perform superficial filler injections too close to the orbital area. If required, fillers can be dissolved.
  • Undercorrection or overcorrection.

We recommend patients to avoid alcohol on the day of treatment. Strenuous exercise should also be avoided for at least 7 days after treatment.

We also do not recommend massaging or excessively touching the treatment area for at least 2 days post-treatment.

In our mid- to late-twenties, volume loss begins and that is when we start to look more tired due to the formation of tear trough.

We may even get more people asking if we slept enough. With age, more areas of the face will experience volume loss such as our cheeks, laugh lines and temples. Starting filler treatments early is great as much less fillers is needed to get nice results, compared to starting when we are older because more fillers are needed. For facial features such as a flat chin, it will not get better with age. In fact, it will worsen with age as we lose bone mass.

The following circumstances may prove unsuitable for wrinkle fillers:

  • Unrealistic expectations.
  • Known allergy to any component of the filler, including lidocaine.
  • Susceptibility to scarring.
  • Bleeding disorders.
  • Active inflammation on the site of treatment.
  • Pregnant or nursing.

We encounter many customers who dislike the fillers they inject at other aesthetic clinics or other countries, but very few from our clinic.

The most commonly injected type of fillers is hyaluronic acid based fillers. Should you not like the result of your filler treatment, a medication called hyalase can be injected to dissolve the hyaluronic acid fillers. The fillers will be gone within a day of hyalase injection. 

Fillers are not harmful to our health, even in the long run.

Fillers are commonly made up of hyaluronic acid which is a naturally occurring substance in human skin. So, it is not harmful to us. The filler only stays at the site of injection and integrates well with skin, replacing lost collagen and providing hydration and support from within.

The next time you need filler injections after your first is at least 1 year later, and we will not look worse if we choose to stop.

For best results, we recommend doing it regularly when you start to notice the effect of the fillers diminishing.

Addiction refers to our inability to resist things which are harmful to us such as alcohol and smoking. Filler treatments are not addictive because we can choose to stop treatments at any time. People look forward to regular filler treatments because it makes us look our best.

In fact, we can choose to stop filler treatments at any time. So we will not be addicted because it is not harmful for our health, but in fact, fillers can help us look our best without going for surgery.
