Types of Acne Scars

The Fractional Erbium Laser (欧洲之星) is a resurfacing laser that delivers energy in a fractional pattern — treating thousands of microscopic columns within the skin while leaving surrounding tissue intact. This fractional approach triggers robust collagen remodelling and new skin growth in the treated columns, while the untreated tissue around each column accelerates healing. The result is a meaningful improvement in atrophic scar depth and skin texture with a significantly shorter recovery compared to full-beam ablative treatments.

The Erbium laser causes minimal thermal damage to surrounding tissue, making it particularly suitable for patients with moderate skin tone (Fitzpatrick III–IV) who require a more conservative approach to avoid post-inflammatory hyperpigmentation. Downtime is typically 3 to 5 days of redness and mild peeling.

The Fractional CO2 Laser (点阵激光) is a more intensive fractional resurfacing treatment that delivers CO2 laser energy to create controlled micro-ablation zones within the skin, stimulating a stronger collagen remodelling response than the Erbium laser. It is most effective for deeper atrophic scars, including boxcar and rolling scars, where more significant tissue remodelling is required.

CO2 fractional laser typically delivers greater improvement per session than the Erbium, but with more downtime, usually 5 to 7 days of redness, swelling, and peeling. Results are typically more dramatic, with a recovery period of around 7 to 10 days involving redness and peeling as the skin renews itself.

The Pico HEXA MLA Laser delivers focused pico laser energy into tiny targeted areas beneath the skin, triggering collagen remodelling and lifting and filling atrophic depressions. 

Because the skin surface is not ablated, downtime is minimal, typically mild redness for 24 to 48 hours, making this a well-suited option for patients who cannot take time off work for a longer recovery. It is particularly effective for shallow to moderate boxcar and rolling scars and works well in combination with subcision for rolling scars.

RF Gold Microneedling (黄金微针) delivers radiofrequency energy through gold-tipped microneedles directly into the dermis at precisely controlled depths. For acne scar treatment, this dual mechanism — physical micro-injury from the needles plus heat from the RF energy — creates a targeted remodelling stimulus at the depth of the scar tissue itself.

It is particularly effective for improving overall skin texture, reducing pore size, and providing a progressive lifting effect on shallow to moderate atrophic scars. Sofwave™ (索夫波) uses Synchronised Ultrasound Parallel Beam (SUPERB™) technology to deliver precise ultrasound energy to the mid-dermis at a depth of 1.5mm, the primary layer where collagen remodelling needs to occur for scar improvement. 

For acne scar patients, Sofwave is particularly useful for supporting collagen production and improving overall skin quality and firmness in scarred areas, complementing treatments such as fractional lasers or subcision. There is no downtime, and it is suitable for all skin types, including darker skin tones.

Subcision is a minimally invasive procedure in which a fine needle or cannula is inserted beneath a rolling scar to release the fibrous tissue bands pulling the skin downward. Once these bands are released, the skin is able to lift more smoothly, allowing subsequent collagen-stimulating treatments such as lasers or RF microneedling to work more effectively.

For rolling scars, subcision is frequently the single most important first-line treatment. TCA Cross is one of the most effective available treatments for ice pick scars, specifically, a scar type that does not respond well to resurfacing lasers or RF energy because the scar extends too deeply. Both are raised scars caused by excess collagen production. A hypertrophic scar stays within the boundary of the original wound and usually improves over time or with treatment. A keloid extends beyond the original wound, may grow larger than the wound itself, can develop months after injury, and is more prone to recurrence. Keloids are more common in darker skin tones and often appear on the upper chest, shoulders, and jawline.

Almost always. Picking introduces bacteria into the pore, worsening infection and inflammation. It can push infected material deeper into the skin, cause surrounding breakouts, and greatly increase the risk of permanent scarring. Professional extraction by a trained therapist with proper tools avoids these risks. If extraction is necessary, always seek professional help.

Those brown or red marks are post-inflammatory hyperpigmentation (PIH), not true scars. They result from inflammation caused by bacterial infection and will fade naturally over time.
