Dr Eva Teh: In Your 30s? It’s Time to Start Aesthetic Treatments

By Dr Eva Teh
Saggy Skin Treatment Singapore

Our skin is a canvas of our experiences and emotions. They say every wrinkle tells a tale – but I’m sure you, like many of my patients, would rather express your life experiences verbally than have them written all over your face in the form of wrinkles.

While it is impossible to avoid having age lines and sagging skin entirely, we can minimise their appearance – and the best way to do so is to start young. For most men and women, this is as early as 30.

In this article, allow me to explain the subtle signs of collagen depletion, factors that accelerate skin ageing and treatments I recommend for those in their 30s and 40s.

Elasticity, Collagen and the Natural Ageing Process

Ageing is an intricate and natural process that affects every individual differently. Two critical components that influence  your skin health and youthful appearance are elasticity and collagen.

1. Elasticity

Skin elasticity refers to the ability of your skin to stretch and recoil like a rubber band. Try pinching any part of your body – does your skin bounce back immediately? This elasticity is primarily attributed to elastin, a protein that maintains skin flexibility.

However, as the years advance, elastin production decreases, leading to reduced skin elasticity. The decline in elasticity can result in the formation of wrinkles, fine lines, and, eventually, skin sagging. This process is usually noticeable in your late 30s, and becomes more pronounced from your 40s.

2. Collagen

Collagen, another vital protein in the skin, serves as elastin’s structural support system. It provides the framework that keeps your skin taut and resilient. Like elastin, collagen production gradually declines with age, leaving the skin susceptible to sagging.

It is estimated that collagen production diminishes by 1% every year after the age of 20. And by the time you reach your 40s, the decline accelerates. The consequences of reduced collagen levels become evident in loose and saggy skin, loss of facial volume, and the appearance of deeper wrinkles.

3. The Natural Ageing Process

Genetic factors influence the natural ageing process and are unavoidable. As you age, the skin’s underlying structure weakens, leading to the thinning of the epidermis (outer layer) and dermis (inner layer).

With ageing, there is a decrease in the number of sweat glands and oil-producing sebaceous glands, too, contributing to drier and more fragile skin.

As collagen and elastin fibres in your skin break down over time, skin repair processes slow down, making it more challenging for the skin to recover from damage and stress. These changes, combined with the effects of gravity, result in a gradual loss of skin firmness, leading to saggy skin.

Other Factors That Can Accelerate Skin Ageing

While natural ageing is inevitable, extrinsic factors such as sun exposure, environmental influences, and lifestyle choices can accelerate your skin’s ageing and lead to premature sagging.

1. Sun Exposure

Excessive and unprotected exposure to harmful ultraviolet (UV) rays is one of the primary culprits of premature skin ageing. UV radiation leads to the breakdown of collagen and elastin fibres in the skin, hastening the appearance of wrinkles, fine lines, and sagging

This process, known as photoaging, can occur without noticeable sunburns, making sun protection an essential aspect of any skincare regimen.

2. Environmental Factors

Exposure to environmental pollutants can cause oxidative stress and damage your skin’s natural barrier. This damage can result in premature ageing, hyperpigmentation, and increased susceptibility to sagging.

3. Smoking

Smoking is not just damaging to your lungs but also your skin. A study saw lower production of type I and III collagen by 18% and 22% in smokers compared to non-smokers. Smoking decreases collagen synthesis, disrupting skin renewal and causing premature skin ageing.

4. Unhealthy Diet

A diet of sugar and processed foods can accelerate skin ageing due to increased inflammation and oxidative stress. This unhealthy combination breaks down collagen and elastin, causing wrinkles, sagging, and a loss of skin elasticity.

Excessive sugar consumption can also cause glycation, damaging proteins and contributing to premature skin ageing.

5. Dehydration

Insufficient hydration can leave your skin dry, dull, and less elastic. Fine lines and wrinkles develop more quickly on dehydrated skin, making it essential to increase your water intake.

6. Sleep quality

Quality sleep is restful and uninterrupted, and provides essential restorative benefits to the body and mind. It involves going through all the different stages of sleep, including deep sleep and REM (rapid eye movement) sleep. During deep sleep, the body repairs and renews skin cells, producing collagen and promoting healthy skin.

However, if you are sleep deprived, these processes will be disrupted, leading to the premature breakdown of collagen and the appearance of tired, sagging skin.

7. Stress and Emotional Well-being

High stress levels trigger the release of stress hormones like cortisol, accelerating the ageing process and contributing to skin problems. Chronic stress may increase inflammation and negatively impact your skin’s ability to repair itself, leading to a tired and aged appearance.

8. Rapid Weight Loss

Rapid weight loss can contribute to skin sagging because the skin needs time to adapt to changes in body shape. When you lose significant weight quickly, your skin may not have enough time to tighten and conform to the new body size.

This results in excess skin that lacks elasticity, leading to sagging and loose skin in various areas of the face and body. The severity of sagging will depend on your age and the extent of weight loss.

Healthier Choices to Slow Down Skin Ageing

Adopting healthier choices and habits can significantly slow skin ageing, Here are some proactive steps you can take to care for your skin and support its vitality.

1. Sun Protection

Protect your skin from photoaging and harmful UV rays by applying sunscreen. I recommend one with at least SPF 30 broad-spectrum protection daily, even on cloudy days.

2. Balanced Diet

Embrace a well-rounded, nutrient-rich diet abundant in fruits, vegetables, lean proteins, and healthy fats. Incorporating foods high in antioxidants, vitamins C and E, and omega-3 fatty acids can help combat oxidative stress and improve skin health.

3. Hydration

Stay well-hydrated by increasing your water intake. Proper hydration keeps the skin moisturised and plump, reducing fine lines and wrinkles.

4. Skincare Routine

Adopt a consistent skincare routine that includes gentle cleansing, moisturising, and using products with ingredients like retinoids and peptides, which can promote collagen production and improve skin texture.

5. Avoid Smoking and Limit Alcohol

Quitting smoking can benefit your skin by reducing the breakdown of collagen and elastin.

6. Quality Sleep

Prioritise quality sleep to allow your skin to repair and rejuvenate. I recommend 8 hours of quality sleep each night to support overall skin health and minimise the appearance of tiredness and fine lines.

7. Stress Management

Find effective ways to manage stress. Stress management can be done by meditation, exercising, or time in nature. Lowering stress levels can help reduce inflammation and promote healthier skin.

8. Gradual Weight Loss

If weight loss is part of your health journey, aim for a gradual and sustainable approach. Gradual weight loss allows the skin time to adjust and minimises the risk of excessive sagging.

9. Regular Exercise

Maintain a regular exercise routine to promote healthy circulation and support skin vitality by increasing blood flow.

10. Avoid Excessive Sugar Intake

Avoiding sugary foods and beverages can help reduce glycation and inflammation, contributing to healthier and younger-looking skin.

Professional Treatments That Tighten and Firm

Fotona

Fotona is a laser technology that can address various skin concerns using a combination of non-ablative and ablative laser wavelengths. Fotona treatments offer a versatile and effective solution for skin tightening and rejuvenation in areas prone to sagging, such as the face, neck, and body.

How it Works:

Fotona employs two laser wavelengths: Nd: YAG and Er: YAG. The Nd: YAG wavelength targets the skin deeply, stimulating collagen production and promoting skin firmness. It works by heating the dermal layers, encouraging collagen remodelling.

The Er: YAG wavelength, on the other hand, focuses on the surface layers of the skin, promoting gentle exfoliation and removing damaged skin cells. This process helps improve skin texture and reverses the signs of ageing.

HIFU

HIFU, short for High-Intensity Focused Ultrasound, is a well-known method for tightening and lifting sagging skin with focused ultrasound waves. HIFU treatments offer a non-surgical alternative to target areas of sagging skin, such as the cheeks, jawline, neck, and brow area.

How it Works:

HIFU technology delivers focused ultrasound energy deep into the skin’s foundational layers without damaging the surface. The ultrasound waves generate precise points of heat known as “coagulation points,” stimulating collagen production and promoting tissue regeneration.

This process initiates a natural healing response, gradually tightening and lifting the skin over time.

Thermage

Thermage is a non-invasive skin tightening treatment. It uses radiofrequency (RF) energy to rejuvenate sagging skin and improve overall skin texture. Thermage is particularly effective in addressing loose and sagging skin on various body areas, including the face, neck, abdomen, arms, and thighs.

How it Works:

Thermage employs radiofrequency waves that penetrate deep into the skin’s layers, heating collagen-rich tissues. The controlled heating process triggers collagen contraction and stimulates the production of new collagen over time.

Increased collagen production leads to a gradual tightening and firming effect, enhancing the skin’s structure and appearance.

Combining Tightening Treatments for 40s and Above

All of the above skin tightening treatments are suitable for individuals in their 30s. However, if you are aged 40 and above, you may benefit more when the treatments are combined.

It is during this phase of life that the signs of ageing tend to become more pronounced, making a combination approach all the more advantageous. By synergising these treatments, we are able to harness their collective power to address a wider spectrum of concerns with precision, yielding optimal tightening results.
